What are your health goals, diet, exercise habits, and lifestyle? Let’s discuss these in-depth during your first appointment. The goal is to create a personalized plan to help you achieve your goals. What's Included: • Medical history and nutritional assessment • Personalized plan to help you achieve your goals • Handy educational materials to support you on your way How to Prepare for Your Appointment: • Bring your Food Diary (2 weekdays & 1 weekend day) • Have recent blood test results on hand (can also be brought to follow-up consultation) • List of all medications and supplements you're taking • Bring a positive mindset as you start your journey toward a healthier you!
Cancellation Policy
I understand that life can be unpredictable, but to respect both our time and efforts, please note the following: 1. Appointments must be canceled at least 24 hours in advance. 2. Rescheduled consultations must take place within the following week. If these conditions are not met, the full price of the missed session will be charged, and no refunds will be issued.
